6. Appointments and Cancellations

Patients are responsible for attending scheduled appointments on time.

Cancellation Policy:

  • Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee.

  • Missed appointments may also incur fees.

  • Repeated missed appointments may result in discharge from the Practice.

7. Fees and Payment

Patients are responsible for all fees associated with services provided unless otherwise covered by insurance.

You agree to:

  • Provide accurate billing information

  • Pay all balances owed

  • Authorize charges for services rendered

Insurance claims submission does not guarantee payment. Any unpaid balances remain the patient’s responsibility.

8. Prescription Policies

Medication prescriptions are issued solely at the provider’s clinical discretion.

The Practice:

  • Does not guarantee prescriptions for controlled substances

  • May require follow-up appointments before refills

  • Does not provide early refills for lost or stolen medications

  • May require drug screening or additional documentation when clinically indicated
